3. COMPLETE: Colonoscopy release & forms
Once you have reviewed all of the presentations and handouts, you must fill out, sign, and return to our office the following forms:
- The Colonoscopy Online Release Form, which states that you have read the colonoscopy information we provided. This form also will help us schedule a convenient time for your procedure. En Español: Forma en Línea del Lanzamiento de Colonoscopy.

- The Insurance Form
Please note: emergencies and cancellations sometimes cause scheduling conflicts with the Endoscopy unit (where colonoscopies are performed). In such cases, we may need to change the time of your appointment. We will try to give you at least 24 hours notice if possible. We reserve the right to change your appointment time, if necessary.
